We accept cash, check, or credit cards (Visa, Mastercard, American Express, and Discover). Our office participates in several managed care plans. Please have your insurance card available for the receptionist at each visit. If we participate in your insurance plan, we will accept the co-pay or percentage at your visit and process your insurance for you. The patient is responsible for all charges and deductibles not covered by his or her insurance plan.
There is also a manditory deposit for all surgical procedures, this $100 deposit will be required when scheduling a surgical appointment and will not be refunded if the patient "no-shows" the visit. Otherwise, this deposit will be applied to the balance due.
In addition, patients will be responcible for a $45 charge on any appointment that is "no showed" (not cancelled within 24 hours of the scheduled appointment time or to any patient who is late to arrive for there scheduled appointment more than 15 minutes). You will not be able to reschedule your appointment until this balance is paid.
